It is important for the family to be informed about the Plan of Care for their loved one, which is essentially a plan that provides direction for individualized care to a person throughout each day and contains the goals and desired outcomes in a client’s condition.

Without a care plan, how do you know if you are being well looked after, are receiving the care you need or want and are being properly charged.
