The objective of Outreach Services, also referred to as In-Home Care, Home
Health Care or Community Support Services, is to allow people to live in their own
homes and communities and to function as independently as possible through the
provision of a variety of care and supportive services.
Many different programs and services are available which are provided by
government agencies, municipalities, homes for the aged, charitable,
not-for-profit groups and for-profit, private organizations. To follow are some
of the services typically offered:
- Personal Nursing Care
- Physiotherapy
- Occupational therapy
- Speech therapy
- Day Programs
- Counseling
- Foot Care
- Friendly Visiting
- Transportation
- Homemaking
- Home Maintenance
- Information and Referral
- Intergenerational
- Programs
- Meals on Wheels
- Respite Care
- Emergency Response
- Congregate Dining
The best place to go to access these services is through your local
Community Care Access Centre.
There your needs will be thoroughly assessed and the most appropriate services
will be recommended. Depending upon your needs, some of the services may be
covered under Ministry of Health funding, regardless of your financial
situation. If you access a service provider directly or wish to retain
additional services, you usually must pay for the cost of the services.
Depending upon your financial situation and the individual service provider,
subsidies may be available.
